If you have received a letter from FTB (Franchise Tax Board), State of
California, “Notice of Proposed Assessment”, it is because they require
substantiation documents to support your filing status as Head of
Household.

If you have declared your children as dependents, please provide the
following:

  • Baby that does not go to school: Medical letter from the Pediatrician indicating the regular medical checking. Please be sure to indicate the complete name of the baby, date of birth, your name and your address.
  • Copies of both social security cards: Yours and your baby.
  • Children that go to school or College: Transcripts from the school or College, Please be sure to indicate the complete name of the child or dependent, date of birth, your name and your address.
  • Copies of both social security cards: Yours and your children or dependents.
  • Rental Agreement indicating the names of your children that are living with you.

Please bring all the above documentation to send it as soon as possible.
